Medical Forms


Medication Authorization Form

This form is required for Nature Camps to administer medications during the camp day and/or during Explorer and TA overnights.  This form must contain current medical information and be signed by the child's physician.

If your child requires more than 3 medications, use Medication Authorization Form B

 

Immunization Certificate

This form must be signed and on file for any child who is not enrolled in school in the United States and any family claiming vaccine exemption. Due to the pandemic, religious exemptions do not apply to the COVID-19 vaccine. Participants with a medical exemption for the COVID-19 vaccine will be required to wear a mask.

 

Teen Adventure Forms

Spoutwood Farm Waiver

This parent signed form must be on file for Teen Adventure participants who are registered for the Spoutwood Farm trip.

 

Appalachian Trail Trip - River & Trail Outfitters Waiver

This parent signed form must be completed electronically through the River and Trail Outfitters website. Registered families will receive an email invitation with a link to complete the waiver online.

Please take note of the following updates to our registration and payment processing system:

Step 1. Online Registration

Click register to set up an online account, register for sessions, and additional services such as, extended day and van service if applicable. 

 

 

At the end of the online registration process you will choose:

  • Check or

  • PayPal invoice

Step 2: Payment

Selecting PayPal invoice will allow you to pay for your registration using a credit card or your PayPal account. You will receive a PayPal invoice via email which will include a link to make a secure payment.

Your invoice will summarize your registration information, total costs, and any discounts that apply.

A minimum deposit of $300 is required. 

Step 3: Forms

 

You will receive a confirmation email which details your registration selections and lists additional forms needed to complete the process.

  1. Print and sign all forms that apply to your child.

  2. Click the "register" button on this page or follow the link provided in the confirmation email to sign back in to your child's account. Upload the signed forms and a copy of your child's COVID-19 vaccination card.

All forms, including COVID-19 vaccination cards must be uploaded by May 15th. For registrations after May 15th, forms must be uploaded prior to your child's first day of camp.
